Forms

For our initial meeting, please complete and bring with you the forms pertinent to our session. For an Acupuncture session in Tucson, please complete the Acupuncture Consent form and the Acupuncture Intake form. If you scheduled and Intuitive Coaching session, please complete the Coaching Consent form. If you scheduled a combination session or are unsure, please complete all three forms. In case of printer mishaps and the like, you are welcome to email me completed forms before our session.  Thank you!
